We are currently accepting new patients.

How to Register

A New Patient Registration can be arranged with our Registration Officer. Please see Surgery Opening Hours on our Home Page, during which when she is not in attendance, the application forms can be completed and submitted for processing the following day.

Passport available to prove eligibility together with a recent utility bill to evidence proof of residence.

We will then ask you to make an appointment to see our Healthcare Assistant for a New Patient Health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

Bringing your Medical Card (NHS Number) will speed up the process of obtaining your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

Remember to bring with you your Passport (Proof of Identity) and a Utility Bill ( Proof of Address) the date of which must not exceed 3 months.
