New Patient Registration

We are happy to register new NHS patients within our practice boundary area.

How to Register

To register with our practice please download, print and complete the relevant form below:



Once you have completed the registration form you will need to return them to the practice.

Please provide two documents as proof of identity to complete your registration or email the completed Registration Form, GMS 1 form and proof of identity to [email protected]

What identification (ID) do we need?
Acceptable Identification so that we can check the details on your form are accurate; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.

PLEASE COMPLETE THE REGISTRATION FORMS THOROUGHLY

(INCOMPLETE FORMS MAY DELAY THE REGISTRATION PROCESS).

Until you have submitted your Registration forms, had your ID checked and contacted the surgery to book your New Patient Health Check you will not be registered at the practice.

Registrations between 10:00am and 15:00pm Monday to Thursday ONLY between these times.

Please bring a Photo ID & proof of address with you to facilitate the speed of the registration.

Acceptable forms of Identification include:

  • Photo ID (Passport, Driving License etc.) 
  • Birth Certificate
  • Proof of address (Bank Statement, utility bills and etc.)

We will then ask you to make an appointment to see our Healthcare Assistant for a New Patient Health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

Bringing your Medical Card (NHS Number) will speed up the process of obtaining your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us.

Registering for Online Services

You can use the Internet to book and cancel an appointment with a GP, request repeat prescriptions and view aspects of your medical record online.

A New Patient Registration can be arranged with our Registration Team.

Registrations between 10:00am and 15:00pm Monday to Thursday ONLY between these times.

By registering for the online services you will be able to complete a variety of different tasks online rather than having to come in to the practice:

  • Book an appointment
  • View past and future appointments
  • Order recent and regular repeat medications
  • View your summary care records
  • Access test results
